Acceptance of Scholarship Agreement
I understand and agree that if selected as a Foundation scholarship recipient, I will communicate with my donor, remain enrolled at least half-time for the academic year, attend the Foundation Scholarship Dinner (if requested to do so), maintain satisfactory status based on the Davenport University standards of progress, and have my photo taken for use in scholarship publications and the website. Withdrawal from a class may result in a decrease in my award, so I understand that I should check with Financial Aid before withdrawing. I certify that the above information is true and complete. I realize that if any of the above information is found to be false, or if I fail to meet the requirements, I forfeit all monies and claims for scholarship consideration. I understand that my completed application will not be returned to me. I understand that completion of this application does not guarantee that I will receive scholarship funds.
